Accepted Insurance Plans: United Healthcare. 

No other insurance plans are accepted at this time.

All copays and co-insurance are due at the time of service.

Pay by: Flexible Spending Account Card (FSA), Health Savings Account Card (HSA), Credit Card, Debit Card, or Cash.

Direct Pay

If you want to receive services and don’t have United Healthcare, you may choose to receive services as “Direct Pay”. This is the process:

While I am a direct care provider who does not submit bills to any insurance provider other than United Healthcare, I do furnish you with the necessary paperwork for payment reimbursement at the out-of-network rate (if offered) from your insurance company. You can also use this paperwork to get reimbursed if you have a Flexible Spending Account (FSA) or Health Savings Account (HSA).

However, you may be eligible to get reimbursed from your insurance provider for all or a portion of your payments, depending on your plan. Please contact your insurance provider for details.
